Wellness User Agreement E-Form

All users of the Penn State Beaver Wellness center will be expected to follow the following rules.  Anyone disregarding these rules will lose their privileges to use the facility.

  1. The Penn State Beaver Wellness Center is available for use by all Penn State Beaver students, staff, and faculty members as well as Alumni who attend between the dates of Fall 2008 through Spring 2012.
  2. All Wellness Center patrons must use their PSU ID card to sign-in/swipe at the Wellness Center Front Desk.  Anyone entering without signing in will be asked to leave the facility.  Repeated violations may result in revocation of Center privileges. 
  3. Wellness Center Patrons must follow PSU policies and Guidelines.
  4. Wellness Center Etiquette:
  • Consider other patrons' rights and privileges while training
  • Spitting into the water fountain is prohibited
  • Placing feet on walls for stretching and/or workouts is prohibited
  • Use of profanity or other inappropriate/derogatory language, talking on cell phones, singing out loud, yelling and screaming, and excessive grunting are unacceptable workout behaviors.
  • Always dispose of trash in the proper receptacles
  • No food or drink is allowed near equipment.  Only water/training beverages in spill proof containers are permitted.
  • Equipment – Follow directions on the proper use of the training equipment and ask for directions when necessary.  Unload all bars and machines when finished with use and return all weights plates, bars, dumbbells, and/or benches to proper racks ( DO NOT LEAVE ON FLOOR).  Please use antibacterial wipes provided to clean equipment immediately prior to and after you use equipment.  Wipe down handles, seating areas, control panels and anywhere else perspiration might accumulate.  Do not bang or drop to the floor weights or dumbbells.  Immediately report malfunctioning equipment to a staff member at the front desk.
  • All personal items (books, coats, etc.) should be kept either in the locker room, or in the storage shelves provided next to the Wellness Center office.  No personal items may be stored in any other areas.  Personal items found in locations other than storage shelves or locker rooms will be considered “lost” items and will be turned into the campus lost and found.  Penn State Beaver is not responsible for lost or stolen items.  It is recommended that patrons do not bring items of value to the Center. 
  1. Building Entrances - All Wellness Center and gym patrons will access the building through the Main Entrance located on the lower level.  The Gym Entrance (upper level) is designated for handicap access and varsity athletics, department and campus special events.  All Wellness Center patrons and guests should exit from the main entrance when leaving the building. 
  2. Locker Rooms – Men’s and Women’s locker rooms are located on the lower level of the building.  Patrons may use these areas to store personal items, change clothes, shower, etc.  The main restrooms are not to be used for changing from street clothes to gym clothes.  All changing of clothes should take place in the appropriate locker room, or in the unisex change/shower/bathroom located next to the Front Desk by requesting access.  Students, staff, and faculty are permitted and encouraged to bring a personal lock to secure items in locker room during workouts.  All personal locks on lockers are to be removed by the patron at the end of each workout.  Please be advised that patron locks not removed on a daily basis may be cut off and removed.
  3. Footwear – Street shoes are not permitted in the Wellness Center, Studio, or Gymnasium.  Sneakers or other appropriate athletic shoes are required in these areas.  No “street shoes”, boots, flip flops, etc. are permitted.
  4. Attire All patrons must wear appropriate gym clothes/work out wear while using the Wellness Center and Gym.  Clothing must be of appropriate size and length to cover undergarments (including sports bras).  Shoes, shorts/pants, and shirts must be worn at all times. 
  5. Building Hours – Wellness Center hours are posted at the beginning of each semester.  Patrons will have access during posted hours of operation.  Access to the facility during “off hours” is limited to varsity athletic workouts supervised by appropriate coaching staff as part of team practice and/or conditioning activities.
  6. Equipment Sign-out – Various equipment (basketballs, volleyballs, etc.) are available for sign out at the Front Desk.  To access these items, patrons will be required to show PSU ID, and sign out such equipment.
  7. General facility scheduling priority may be given to academic classes, varsity sports, club sports, intramural/recreational sports, student organizations, and reservations as approved for campus departments.
  8. Music – Music in the Wellness Center is controlled by the Front Desk.    The use of personal audio devices is permitted (ex: iPods, mp3 players, etc.). 
  9. Patrons may sign in one accompanied guest.  The sponsoring individual will be responsible for, and in the presence of, their accompanied guest at all times while using any of the facilities.  Accompanied guests under the age 16 are not allowed in the Wellness Center and may use only the studio and gymnasium.  Access to such facilities will only be authorized during the time each facility is specifically designated as being open/available for use.
  10. Patrons are not permitted to kick/catch/ or throw balls in the studio yoga room.  
  11. Any violation of the above rules and regulations may be grounds for loss of privileges, or removal from the facility. Front Desk attendants (student employees and supervisors) have the authority to request patrons to leave the facility. Serious offenses will be referred to the appropriate authorities. If asked to leave, please comply in a timely manner or Penn State Police may be called.  If you feel you have been treated unfairly, please contact the Athletic Director, Coach Andy Kirschner ( ack121@psu.edu), the Assistant Athletic Director, Marcess Williams ( mew23@psu.edu ), or the Athletics Coordinator, Vanessa Jackson ( vej1@psu.edu ) or stop by their offices on the upper level.

Waiver of Liability (For Students/Faculty/Staff)

Patron (or parent/guardian of minor guest) understands that The Pennsylvania State University, Beaver Campus, through its Wellness Center, offers the use of the gym and fitness equipment, which can be potentially dangerous and involves physical stress.

Patron further understands that physical injury may result from the use of this equipment or involvement in any physical activity in the Wellness Center and gym.

Therefore, it being the intent of the Patron to hold The Pennsylvania State University, Beaver Campus, its trustees, officers, agents and employees harmless, Patron hereby releases, waives and forever discharges the same from any and all liability for injury suffered by Patron related to the use of the Wellness Center and gym. Patron understands that he/she is using the equipment at his/her own risk. We recommend that patrons become oriented with the equipment prior to use. Patron is responsible for knowing the rules of the Wellness Center and gym and failure to comply with the established rules, will result in possible suspension or termination of privileges.

Only authorized employees of the University will review the information on this form. This form will not be disclosed to third parties without your authorization, except as required by subpoena or other legal process.
